  WP 4 - Integration of Risk and Reliability Formulations

Objectives
Risk assessment and management with special consideration to the problem of integration of the various disciplines such as SRA, QRA and risk assessments concerning human and organisational errors.

At present such studies are mostly performed in a non-interactive environment and there are many situations, in which this is clearly not optimal. Typical examples hereof are risk analysis in feasibility studies and for inspection and maintenance planning, where the risks addressed involve a variety of human and organisational factors, operational errors, fires and explosions and structural failures. Ways of conducting a more integrated analysis and of using computational techniques developed in one of the areas for applications in the other will be explored.

In consistency with the identified approaches for integrated risk assessment a framework for risk management will be identified considering in a generic sense the typical risk assessment/decision problems as they occur in industrial applications.

Task 4.1 – Methods for Integrated Risk Assessment
Task leader: COWI

The emphasis in this task will be directed towards the identification of appropriate methods for seamless integration of QRA and SRA together with risk assessments in regard to human and organisational errors as required in accordance with the typical application problems identified under task 2.1, 2.3 and 5.1. Among others causal probabilistic networks such as Bayesian Probabilistic Networks (BPN) and Bayesian influence diagrams will be investigated as possibilities for this. The results will be collected in a report.

Task 4.2 – Risk Management Framework
Task leader: WSA

The objective of this task is to develop an outline framework for risk management of major infrastructure and industrial systems. The framework which must be consistent and aligned with the Integrated Risk Assessment framework would cover the following aspects:
 • Arrangements to ensure positive safety culture and organisational learning
 • Safety policy and Safety Management Systems covering:
 • Organisation, role definition and competency management
 • Work control/procedures and change control processes
 • Quality assurance and quality control
 • Arrangements for dissemination of safety related information/notices
 • Principles and requirements for safe operation, inspection and maintenance
 • Processes for Hazard Identification, Logging and life-cycle Hazard Management
 • Use of the integrated methodology for risk analysis and risk-based decision-making (from Task 4.1) in support of the above
 • Identification and implementation of risk reduction measures involving inherent safety, prevention, protection, mitigation and control measures
